With Caddy set up as a reverse proxy as I have done, do you still need to check the "allow remote connections to this server" box on the Jellyfin server. I assumed this would not be needed as connection from caddy to the server is local, but if I uncheck it external connections are refused.

You still need that enabled. Caddy will forward the IP of the original requestor to Jellyfin.
