I tried renaming the file to just:  D01T03 - Ridley Scott Introduction.mp4, but it did not help.
I even tried bob.mp4, but that didn't work either.

Then I thought I'd try the File Suffix approach, rather than folders.  Here are my findings:

The following File Suffixes DID NOT work:
- Trailer:          D01T03 - Ridley Scott Introduction-trailer.mp4
- Other:            D01T03 - Ridley Scott Introduction-other.mp4

The following File Suffixes DID work:
- Sample:           D01T03 - Ridley Scott Introduction-sample.mp4
- Scene:            D01T03 - Ridley Scott Introduction-scene.mp4
- Clip:             D01T03 - Ridley Scott Introduction-clip.mp4  (displays as featurette)
- Interview:        D01T03 - Ridley Scott Introduction-interview.mp4
- BehindTheScenes:  D01T03 - Ridley Scott Introduction-behindthescenes.mp4
- Deleted:          D01T03 - Ridley Scott Introduction-deleted.mp4
- DeletedScene:     D01T03 - Ridley Scott Introduction-deletedscene.mp4
- Featurette:       D01T03 - Ridley Scott Introduction-featurette.mp4
- Short:            D01T03 - Ridley Scott Introduction-short.mp4  (displays as featurette)
- Extra:            D01T03 - Ridley Scott Introduction-extra.mp4  (displays in Special Features with no tag)

The fact that I can get this feature to work with some folder names and some file suffixes tells me that this Jellyfin feature is working for the most part. 
It's just only some of them are not working.

I'm assuming you rescanned after renaming.

I know I asked about permissions and you said you didn't think it was. Can you show me what the permissions are for these files? Are the files on Linux or Windows? A permissions issue explains your problem. Or a corrupt file. If it isn't either of those, I have no idea what is going on.

Yes, I rescanned after renaming.
I also exited out of the movie selection back to the main "Movies" folder, and then choosing the movie again.

I'm doing this on Windows.

Definitely not a permissions issue because I can use the same file/folder, rename it to one that I know works, and it works. It's only when I change it to the ones that don't work, where it doesn't work.

Just curious... I see you run Jellyfin on Linux.  Can you see the problem there as well?
Do you also have a Windows machine where you can try what I have documented?  It's very easy to replicate.  And I see other users are having the same issue.

I'm just wondering if it is a Windows issue only.

I do have a test setup on my Windows desktop. Unfortunately, I am house/cat sitting for my parents during the week and won't be able to access my desktop until Thursday. I will set a calendar reminder to test this when I get back to my house.

I don't have any actual extras to test with, but I could use some of my generic test videos and make them "extras". I don't have access to my server at work and my time at home is a bit short at the moment. Probably won't have time to setup test until Thursday on my Linux system, which is when I would be testing on my Windows system as well.

Yes, you could even use an actual episode as an "extra" by copying the episode into one of the folders (like "featurettes", "trailers", etc).

Regarding, Thursday or later:  sounds good.

The only thing I've been able to figure out is that if you have a single trailer file, it won't show up as a "Special Feature".  You have to click on the trailer button next to the play button.  I believe that is an intended behavior based on the documentation.

I couldn't figure out why "other" extras don't work.

Ignore the special feature thumbnails. I just used some small music videos as fake extras.

Thank you for the follow-up and sharing that information.  I didn't realize it would show up under "Play Trailer".  It wasn't documented that way (https://jellyfin.org/docs/general/server/media/movies/), and I assumed it would just show up under "Sepecial Features" with all the other extras.

Here's what I can confirm now:

When using the web client:
  • When you have a folder/file for a trailer, I can see the trailer when I click the "Trailer" button next to the "Play" button.
  • When there is no trailer folder/file, clicking the "Trailer" button will play a video from YouTube.

When using the Roku client:
  • When you have a folder/file for a trailer, I can see the trailer when I select "Play Trailer".
  • When there is no trailer folder/file, the "Play Trailer" option is not visible.  I can't even play a trailer from YouTube.  But I believe this would be a Roku Jellyfin Client issue.  I can take it up with that project's developers.

And here is an updated summary of what does NOT work.  I can live with this  Smiling-face  but not sure if it either needs to be fixed later or if the documentation needs to be updated.

Folders:
  • clips
  • other

File extensions:
  • other
