When I have "Enable 'Play To' DLNA feature" enabled I get 2 devices showing up in my active devices called "XBOX DLNA 10.8.13".

I don't have the "Enable DLNA server" option enabled so I didn't think anything could connect without an account?

When I disable "Enable 'Play To' DLNA feature" the XBOX no longer connects. My brother has the only XBOX in the house but it connects almost instantly after I enable the Play To option so I think it's just auto-connecting and not my brother doing it himself.

My main worry is if he can actually see anything or play anything like this? and if it's possible to stop it connecting somehow without disabling the Play To option?

I'm assuming "Play to" is something you'd initiate from a browser and "push" to the Xbox's. The Xbox's are probably responding to the server as available "targets". But my understanding is that the Xbox's can't initiate playback or see the Jellyfin server's content. Only when you enable the DLNA server.

That does seem to be the case as far as I can tell. disabling the "play to" feature stops google cast support. Just find it concerning that the Xbox's can even connect with no account info.

They're not "connecting". They're just "targets" that can be cast to. You're logging in on the browser that initiates the playback. The unauthenticated Xbox's cannot initiate playback from the Xbox, unless the DLNA server is enabled. But that is one of the short comings of DLNA and is also the reason that DLNA is LAN only and cannot be used over the Internet.

Makes sense but Chromecasts only show up when I explicitly cast to them, the Xbox seems to connect itself without anyone selecting to cast to it. Is this just some kind of discovery thing Xbox's do you think?

Probably.
