What does this mean from the logs?

[2024-05-25 20:17:36.558 -05:00] [INF] [27] Jellyfin.Api.Auth.CustomAuthenticationHandler: "CustomAuthentication" was not authenticated. Failure message: "Invalid token."

How do I fix it?

I get this running Jellyfin 10.8.13 on an Intel Mac running MacOS 14.5

This means one of the client keep an outdated login token and tried using that token to login. The most common reason of that is a browser caching old server info while logging in to the new one. If you are the one make this login you just clear your browser cache
