Season & Episode Numbers in Title

I upgraded to 10.11.1 and all went smooth (about 25min). Did a full library scan which took 24 hours and after that system working.
Couple small issues I noticed :
* Not all watched episodes / Movies was flagged as watched after upgrade.
* Newly scanned episodes include the Season & Episode in the Title.


I am not sure if I'm missing a setting somewhere that take the file name as the Title or ***arr is pulling the wrong info into the nfo file.
Anybody having the same issue or know what I'm missing?

Same here.  do a Refresh Metadata and select Search for missing metadata, you will see a message saying Queued but just click back at home and it will show up.
