SOLVED: Jellyfin repeating episode names

I'm running into my first naming issue with Jellyfin. I have a series with the correct naming that has episodes 1-16.

Jellyfin is pulling the correct files (when I watch episode 1, it is in fact episode 1) but the naming is wrong. Jellyfin has named episode 1 with the episode 3 name, episode 2 with the episode 4 name, etc, etc, up through episode 13 and then the final 3 episodes (14, 15, 16) are the correct name. 

Both of the "embedded titles" options in the library have been unchecked since library creation.

Was a metadata source issue. Had to add TheTVDB plugin, uncheck other metadata sources from library, delete file from Jellyfin, restart server, rescan library, lock metadata for season, re-add metadata sources to library.
