Sounds like something might not be firing in the background. Have you checked your logs? That should get picked up by an automated process, but I'm not sure which task. Look at your scheduled tasks, see about when they run, and scan your logs to see if there are warnings or errors that might key you in on if there's an issue that could be causing these to not get added. If it's a new occurrence, there might be a new problem with your setup, config, or even a bug that has been introduced. Keep an eye on it. I haven't noticed, but my attention has also been elsewhere.
