Just noticed that the video is Dolby Vision 7.6. It sounds like it is trying to software decode the video and isn't able to keep up.
Dolby Vision 7.6 is not well supported outside of bluray players. Your options are to force transcoding, so that Jellyfin will tone map it, or convert it to HDR10.
