It may not be finding any new media to scan. When your media is incrementally scanned, those can be super quick. I tend to like refreshing by going to Dashboard > Libraries and scanning the individual library. This allows you to select replacing all metadata.
If something is still wrong -- and it's just a few items -- fix manually by removing ALL metadata from the series or movie itself. Click the kebab on the movie/series and click Identify. Enter the correct ID in the corresponding field for the metadata provider of your choice.
If it's a LOT of content, you may need to review your folder structure against the docs.
