addressing point 2 first - I could no instance of 'BetterMix: Falling back to Native Instant Mix' anywhere in my log files after doing some testing.
however - your mention of the playlist answered my unasked question of where the generated lists of songs were going - I found the daily mixes and discover playlists etc. in the Music \ Playlists section (of course) - but because I have hundreds of albums with playlists and the playlist names can only be shown in alphabetical order, I never noticed the BetterMix generated playlists.
As for the generated playlists.. they were very mixed and not sounding similar either.. eg:
Daily Mix 1, song 1: On the Beach (Chris Rea), song 2: White Wedding (Billy Idol) etc..
Daily Mix 2: Esther Be the One (ZZ Top), song 2: Don't Worry Be Happy (Bobby McFerrin) etc..
I tested setting the noise parameter to 0 and saved it, then tested Instant Mix by playing Devo - Mongoloid. It DID play the exact same song (thought a version on a different album), then for the third song it played '60 Miles an Hour' by New Order, for the fourth song it played 'Catch My Fall' by Billy Idol.
When I installed the plugin I installed version 0.4 direct, I have not previously installed version 0.2 or 0.3
I will send you the logs from when I played the first song (Devo - Mongoloid) to investigate. Thank you.
