I ended up working on this for hours trying all combos of settings. I was fairly convinced it was not Jellyfin and it was the Firecube or my receiver. Tried everything but always the same, with TrueHD and just passthrough Jellyfin video stutters really bad, as in a frame every few seconds. Ironically the sound itself seems to be fine.

In trying to solve this I read Plex now will handle TrueHD passthrough. I tried Plex with no settings change and sure enough it worked fine. So I guess my fix is to watch the 4k TrueHD stuff on Plex unfortunately.
