I'm having the same exact issue (~24k items in my Home Videos library), except that since I didn't care too much about the existing library (because everything had its own folder already) I just downgraded to 10.10.7 and recreated the library; everything is back to working.
Before doing that I'd also deleted and recreated everything on 10.11 (multiple times...) and the problem kept reappearing, so I know this is a 10.10 issue
Looks like this is being tracked in https://github.com/jellyfin/jellyfin/issues/15141 in case you want to sub there too. I tend to agree with the folders possibly being the root cause, because I tried to add each folder individually to a test library and that never had an issue
