    1. How can I make the image for this extra video not be the same as the cover for the movie? It's confusing. Plex just shows a thumbnail.
    2. How can I make the extras show as horizontal thumbnails as opposed to 1:1.5 cover images?

    Yes. But I have it added inline in the same folder as the movie as per the documentation. So it is named

    Move it to an extras folder. You most likely have a poster image in the main folder that Jellyfin is using for both.
