If you're refering to cover art that have the movie's title over it, you may have to manually use the built-in image search feature to look for one that has that (if the auto-pulled cover didn't have the title).
The top choice for what is pulled may not always include the title name in the cover. Some people prefer one or the other. It's up to the metadata sources to choose which versions they decide to host/provide.

BUT if I change it into one with text on the poster and press save the old image still appears without the text
Even after hours. What could be wrong? I run new macOS version 14.5 with Jellyfin 10.9.4

Did you try clearing out the client's cache?

Just tried deleting the Jellyfin app on my Apple TV and iPhone. And old thumbnails are still showing

Deleted client cache within Jellyfin app on iOS just now. Same result

The images that Jellyfin uses is dependent on the metadata source. I'm not entirely what criteria Jellyfin uses for selecting images where there are multiple choices. But there will be versions of these images without the movie title.
If Jellyfin happens to choose the image without the movie title you will have to "edit images" and manually select one with the title.
