You buy a domain from a domain registrar, like Cloudflare. You provide your public IP address to Cloudflare and when you use your domain name it will direct the request to your public IP.

If you don't want to pay for a domain name, there are free services like duckdns that will give you semi-custom domain names.

I have created one. Now, how can I connect it to my jellyfin server and will I have to do any port forwarding?

You will need to port forward port 8096.

Wherever you have your domain at, it should ask you what your public IP is. Once you've provided that and setup port forward, that's it.
