    Just got a Roku Ultra and I can confirm the same behavior. After setting up with 5 width and height in the trickplay settings I can only see the thumbnails when a video is being transcoded. My whole library is HEVC so I can just "Disable HEVC" in Jellyfin on the Roku to force transcoding of all content, but that seems like a step in the wrong direction. Really wish this worked for direct play.

    Update:
    Been playing with the trickplay files for several weeks now. Thanks again to @HokieRulz for the guide to set the Tile Width and Tile Height to 5x5.

    The video thumbnails mostly work on recorded TV but there are several instances when a recorded show starts with thumbnails for the first few minutes...then nothing! There is no rhyme or reason to why. I though it was specific stations but future recordings had thumbnails throughout the entire show - so who knows what's going on.
    In some rare instances, the thumbnails are present for almost the entire show then disappear for the final 10 or so minutes.

    I am going to do more experimenting using both NPVR and Jellyfin to record shows to see how that may affect the trickplay thumbnails.

    Looking for any updates or progress on this. I haven't changed the dimensions of my images yet as it seems that doesn't guarantee that the Trickplay thumbnails will show on the Roku. Thanks in advance.

    Version 3.0 of the app should have better trickplay support. Should be out soon-ish.

    If you want a preview, look through the Roku store for the "Viv" app. Our Roku dev has been working on Viv on the side as a sort of Roku re-write.
