Metadata is pulled when:
1) The content is scanned in, either through real-time monitoring or via scheduled process.
2) When you manually scan an item* using "Search for missing metadata" or "Replace all metadata" options.
3) On scheduled refresh, which is set per library as Never or 30, 60, or 90 days.
*An item can be a library, movie, show, or episode. The option to refresh metadata or scan a library both have the options described.
So likely what happened is that the episode entered your collection so quickly that TMDB was not yet updated, so it labeled it as "Episode 4". In 30-90 days (or never, depending on your library setting) the title would be updated based on your metadata provider's information at the time.
To fix this, go to the Episode page in your browser as an admin, click the kebab (three vertical dots), and click Refresh metadata. Select either "Search for missing metadata" or "Replace all metadata" from the drop-down and then click Refresh. You will likely need to refresh the page or go back Home and pull the episode up again to see any changes. This should populate the new data.
