Also facing the same issue here. I've had Intel QuickSync working well with no issue for months and all of the sudden I get the "A fatal error was encountered in the HLS stream" on most of my movies. Since no settings or config were changed recently, I decided to do an update and see if that would help.
Unfortunately I winded up with the broken migration.xml issue that caused my disk use to swell to 100% by inflating my daemon and system logs. Each were 20GB+ in a few minutes. While now I am on 10.9.10 and all is operating mostly well, I'm still stuck with the HLS issue. After a few days of troubleshooting the other issues, I'm kind of hitting a block here. I'd appreciate any help to see what might be causing the error.
Also note that I tried to turn off transcoding off, but was getting the same error.
