    Hey, I think I have a similar issue? My extras are not using the file names at all, regardless of library settings and metadata refreshes.

    I think I had the "Prefer embedded titles over filenames" enabled when initially setting up the library, but disabling it is making no difference on any "Replace all metadata" scan for the library, movie, or extras item.

    I can see the video metadata in the files themselves using other programs - the names Jellyfin is using and not updating is definitely from the metadata embedded in the video files.

    I'm on Linux and have had these files organised since they were on a Windows machine, so permissions issues could be a thing - but I don't see any such issue in the Jellyfin server logs, playback works fine, Jellyfin can read the filename when I dig into the video's media info pop-up in Jellyfin library view, and so on and so on. And if it's impacting the OP and they don't have that same changing-OS history like mine, then it could just be a problem with the library metadata scanning.

    So, right now I'm digging around for ways to bulk-update video metadata based on filenames, since that does seem like the best option for proper library organisation in the long-term... but this does feel like there's a problem with Jellyfin's metadata scanning from this situation.

    Edit: Making a new library with the desired settings from the start AND renaming the parent directory of all of the old library's content so it appears to be "new" to the Jellyfin server allows the content to use filenames as the content titles as intended.

    Making a new library with the desired settings from the start but NOT making the files appear new is not enough - Jellyfin will recognise files and apply their old metadata when scanning that new library. Gotta have something change in the file path to force Jellyfin to treat the content as new to make this workaround work.
