Everything came with the 10.9.X, could it be a new feature that is causing this?

Are the disks local to the server? Do you use real-time monitoring? Maybe try disabling that and running a scan.

I have a little PC with 2 very large hard disk's connected throught USB, I don't use any real-time monitoring, but I disabled windows defender real time protection, just in case.

I am deleting all the .nfo of the shows and anime that have this problem and adding them one by one.

I just saw that some of my anime's were fucked (also being reescaned), the season 1 episodes went to OVA's for some reason and cases like that. So I move them temporaly to other folder that jellyfin can't see and reescaning all again, then I am doing the steps that I described above.

I'll write if it works, but that started to happen just right after the last update.

I am not alone, in this post, the user reports the same issue that me:

So seems to be that 10.9.4 is the problem
